The annual CHIC/Parent Club Champion of Health Award is sponsored by the Orthopedic Foundation for Animals (OFA), and is intended to recognize a dog that has had significant accomplishments in the ring, in the field, in performance events, as a producer, or as a working dog, AND has the full complement of health testing appropriate for its breed. The award consists of an engraved acrylic trophy, a certificate, and a $100 donation to the AKC Canine Health Foundation breed specific donor advised fund in the name of the honored dog. The award is announced and given during the ASSA national specialty show.
Nominations are sent to the ASSA Health Committee that will select up to 3 dogs to submit to the OFA for the final selection. Each nomination should include a brief write-up of the dog’s accomplishments as well as a record of its health certifications. Additional health screenings beyond those needed for CHIC status are not required for nomination, but will enhance the overall nominee’s qualification for the award. Participation and titles in more than one venue are encouraged. Special accomplishments of offspring and accompanying health clearances/CHIC numbers will be an additional consideration.
The nominations must be presented to the OFA a minimum of six weeks prior to the specialty event, in order to allow time for engraving, etc. The OFA reserves the right to withhold the award based on a lack of suitable nominees.
Complete the following information for each nominee and enclose proof of non-OFA health clearances and achievements. Please only one dog per form. Additional information about the nominee is encouraged! Use the back of the form or additional pages. The deadline for submitting this form is January 15, of each year.
